To personalize your SMART Load Wallet account, a personalized SMART Money Card is required. Your Load Wallet account will be automatically personalized 7 days after you have received your SMART Money Card. To apply for a SMART Money card, simply visit the nearest SMART Store and bring the following:
- 1 valid and unexpired ID
- Driver’s license
- Passport
- Major credit card with picture
- Laminated company ID
- Professional Regulation Commission (PRC) ID
- Laminated SSS ID
- Taxpayer’s Identification Number (TIN) card with picture
- Un-expired school ID’s (for applicants between 18 – 20 years old)
- Postal ID
- Retailer SIM
Kindly accomplish the SMART Money Application Form (SMAF) and pay a one-time processing fee of P120. The personalized card will be delivered to your preferred address after seven (7) days if within Metro Manila or fourteen (14) days if provincial.
Note: You may also opt to pick-up the personalized card at your preferred Smart Store, however the P120.00 processing fee still apply.
