For defective SIM card within the 7-day warranty, just present the SIM for replacement and the original copy of the proof of purchase (OR) at any SMART Store. Replacement is free of charge.
For defective SIM card beyond the warranty or requests for SIM upgrade, present the SIM that will be replaced and pay a SIM replacement fee of P40 for SMART Prepaid and P30 for Talk 'N Text.
For lost or stolen SIM card, submit the following documents/requirements to any SMART Store.
- Duly notarized Affidavit of Loss
- Two (2) valid Proof of Identification
- Any proof of ownership of the lost/stolen SIM card (PUK slip, SIM casing, personalized Smart Money card linked to the lost/stolen SIM card)
- SIM replacement fee for P40 (SMART Prepaid) and P30 (Talk 'N Text).
Please note that availability of SIM card replacement retaining the same mobile number is subject to system availability. If unavailable, kindly purchase a new SMART Prepaid or Talk 'N Text SIM card to continue enjoying our prepaid services. Remaining airtime balance from the previous number will not be transferred.
